Pregnancy needs roughly 1,000 mg of extra iron. Most women don’t start with that in reserve — and a standard blood count won’t show it until stores are already empty.

What Doulas Tell Us

You See It First. But You Can’t Order the Labs.

You’re with your clients more than anyone on their care team. You see the 3pm crash, the breathlessness on the stairs, the ice she keeps chewing, the tears at week ten postpartum. And you hear the same answers over and over:

✓“It’s normal to be tired when you’re pregnant.”
✓“Your hemoglobin is a little low — take your prenatal.”
✓“Nobody checked my ferritin.”
✓“The iron pills make me so sick I stopped.”
✓“The hematologist can see me in seven weeks.” (After her due date.)
✓“They sent me home after the hemorrhage with a bottle of pills.”

It isn’t your role to diagnose — and it shouldn’t have to be your job to fight for a ferritin test. That’s where we come in.

Why It Matters to the Birth You’re Supporting

Low Iron Can Quietly Rewrite a Birth Plan

Iron deficiency is the most common nutritional deficiency in pregnancy, and it rarely announces itself. Left alone, it can affect far more than energy:

✓Many home-birth midwives and birth centers have a hemoglobin cutoff — miss it at 36 weeks and the plan changes
✓Anemia going into labor leaves less reserve if there’s heavy bleeding, and raises the chance of transfusion
✓Untreated iron deficiency is linked to preterm birth and low birth weight
✓Postpartum, it’s associated with exhaustion, low mood and postpartum depression
✓Oral iron can take months to work — time a third-trimester client may not have
Simple for You. Fast for Her.

How the Doula Referral Pathway Works

Two minutes from you. A hematologist for her within days — not weeks. Her OB or midwife stays in charge of her pregnancy; we handle the iron.

Clients Who Should Have Their Iron Checked

During pregnancy — any of these is a good reason to refer:

✓Hemoglobin flagged low or “borderline” — or ferritin never checked
✓Can’t tolerate iron pills, or has taken them for weeks without improvement
✓Exhaustion, breathlessness, dizziness, racing heart, or restless legs
✓Craving ice, starch or other non-foods (pica)
✓Heavy periods before pregnancy, pregnancies under 18 months apart, or twins
✓Planning a home birth or birth center birth with a hemoglobin requirement
✓Declines blood transfusion for personal or religious reasons

After birth — especially:

✓Postpartum hemorrhage, heavy blood loss, or a cesarean birth
✓Still exhausted, breathless or foggy beyond six weeks
✓Heavy hair shedding, low mood, or feeling “not herself”

Frequently Asked Questions

Premier Hematology infusion suite

Can a doula refer a client to a hematologist?

Yes. Premier Hematology does not require a referral from a doctor, so a doula can refer a client directly with her permission — or the client can call us herself. We coordinate with her OB or midwife once she’s been seen.

How quickly will my client be seen?

Most clients are seen within 24 to 48 hours at one of our 14 New York centers. We call her to schedule as soon as a referral comes in, and we verify her insurance before the visit.

Is IV iron safe during pregnancy?

Modern IV iron formulations are widely used in the second and third trimesters, when oral iron fails or there isn’t enough time for it to work. It is generally avoided in the first trimester. Our hematologists decide case by case, in coordination with her obstetric provider.

Can IV iron help a client meet a hemoglobin requirement for a home birth or birth center?

Often, yes — if it’s started early enough. IV iron typically begins raising hemoglobin within one to two weeks, with the full effect at around four to six weeks. The earlier in the third trimester she’s seen, the better the odds. Eligibility decisions stay with her midwife or birth center.

Is IV iron safe while breastfeeding?

Yes. IV iron is compatible with breastfeeding, and we see clients at any point postpartum. Babies are welcome at postpartum visits.

Will insurance cover it?

We accept most major insurance plans, and iron infusions for documented iron deficiency are commonly covered. Our team checks her specific coverage before her appointment so there are no surprises.

Will you let me know my client was seen?

With your client’s permission, yes. Her medical details stay private, but we can confirm she’s been seen so you can keep supporting her. Her full plan goes to her OB or midwife.

Can you present to our doula collective or training program?

We’d be glad to. Our team offers short in-service sessions on iron in pregnancy and postpartum, plus referral cards you can hand to clients. Mention it in the form below or call (718) 866-3037.
